What should I bring to my appointment?
Please bring your ID, insurance card(s) and a list of your current prescriptions.
Do you take insurance? If so, what kind do you accept?
Yes we accept the following insurances:
- United Healthcare PPO
- Blue Shield PPO
- Blue Cross PPO
- Aetna PPO
- Cigna PPO
Do I need a referral to make an appointment?
If you have insurance that we accept you do not need a referral. If you have a different insurance from what we accept above, you will need a referral in order to be seen and have the services paid for. Otherwise you will be responsible for all charges associated with your visit.
Are you accepting new patients?
Yes we are accepting new patients, however we are not accepting new Medi-cal patients.
Do you accept walk-ins?
No, we are by appointment only.
Do you have a fee for a missed appointment?
Yes, if you do not cancel your appointment at least one business day in advance you will be charged $50.00 for a regular appointment or $75.00 for a procedure/surgery appointment.
What forms of payment do you accept?

- Accepted Insurance
North Valley Dermatology Center requires payment in full for any patient portion owing at the time of
your visit. This includes co-pays, deductibles and any other portion that your insurance specifies as
patient responsibility. We will estimate the cost of your visit as accurately as possible, however there
may, in some circumstances be an additional balance owed.
